This project will identify risk factors that lead to preventable patient harm in children on invasive home medical therapies by interviewing parents of children who are on home mechanical ventilation and home parenteral nutrition and by visiting them in their homes. We will use human factors and qualitative research methods to identify these risk factors and to develop a preliminary model of causality of harm. We will then use the findings of this study to develop interventions to mitigate and/or eliminate preventable patient harm in children on home mechanical ventilation and home parenteral nutrition that we hope to generalize to all children on invasive home medical therapies.
